Do you offer in-person or virtual ?
I provide both in-person and virtual appointments. Virtual appointments will be held through a secure portal link.

Do you accept insurance?
Right now I am in-network with Univera and Martins Point. All other insurance would be considered out of network/private pay. I am able to provide a super-bill to submit to the insurance for partial reimbursement (plan depending).

when is payment due?
Payment is due the morning of - unless previously discussed. Payment is done through our secure elecontric medical record.

what form of payment do you accept?
HSA/FSA cards, all major credit cards, cash and check are accepted.

What if I need to cancel my appointment?
It happens! Sickness and family emergencies happen. If you need to cancel I kindly ask that you give me 24/hr notice so that you are not subject for the late-cancelation fee.

what should i prepare to bring for my first appointment?
Just yourself! During our consultation I send over a portal invitation to complete all initial paperwork prior to your appointment. Which includes entering your prefered payment information and your insurance information. If you want to bring anything else, i.e a journal, a list of questions/items to discuss you are absolutely welcome to do that.
